I am the owner of the above described animal and have the authority to execute this consent.

To the best of my knowledge my animal has no diagnosed allergies to vaccines.

My animal has had no recent occurrences of abnormal coughing, sneezing, vomiting, diarrhea or runny eyes.

I understand that the Humane Society of Broward County uses only the highest quality of vaccines available, however a vaccine reaction is possible, yet rare.

Should my animal become ill due to a vaccine I will not hold the Humane Society of Broward County, South Florida Lending Hands its affiliates or employees responsible, and agree to take my pet to my local veterinarian and assume all responsibility for any cost incurred.

I understand that this is not a full and complete exam and for the overall health of my pet a full and complete exam should be performed yearly at my local veterinarian.

IF this is the first vaccine your pet has ever received, they will need a follow up booster in 3-4 weeks (It will be $30.00) to be fully protected.

To address this critical need, South Florida Lending Hands and Petco Love are partnering to make pet vaccines free and accessible in an effort to ensure the health and wellness of our beloved pets.
We're proud to be partnering with Petco Love in their commitment to 1 million free pet vaccines and help end life-threatening diseases in pets.
Top diseases preventable by vaccines are impacting our pets nationally. Parvovirus, distemper in dogs, and panleukopenia in cats are seen too often impacting our most vulnerable pets. That's why we're taking action to help keep your pets healthy and providing free pet vaccines directly to you.

South Florida Lending Hands will provide free vaccinations for South Florida area pets. The free vaccines are made possible through Petco Love's new national vaccination initiative, which is providing 1 million free pet vaccines to its existing animal welfare partners, including South Florida Lending Hands, for family pets in need.
